U.P. Dental Council is established under Dentists Act 1948. U.P. Dental Council is registered & recognized by Dental Council of India. This council is authorized to do the Registration of B.D.S., M.D.S., Dental Hygienists, Dental Mechanics as per the schedule of Dental Council of India.

Renewal of all the respective registrations will be done every year before 1st April as per section 39 of Dentists Act 1948.

UP Dental Council Uttar Pradesh Registration form:

Go to the official website,click on the Download tab in home page,then choose registration form.
Enter the following details to fill the application form.
Enter Your Aadhaar Number
Enter Your Mobile Number
Enter Your Date Of Birth
Enter Your First Name
Enter Your Middle Name
Enter Your Sir Name
Then Fill the remaining details,to complete the registration form.

Update Mobile No./ EMail Id Online Update Mobile No./ EMail Id/ Aadhaar No. Application :

Enter following Authentication’s Details
** Select Type of Registration
** Enter your Registration No.
** Enter your Date of Birth as entered within Registration Certificate.
** Click on Authenticate button.

Registration Fees for UP Dental Council Uttar Pradesh Registration:

1. Dentists Registration + Smart Cardv- Rs. 3,950/-
Dentists Registration Other State + Smart Card – Rs. 4,050/-
2. Dental Hygienists / Mechanics Registration + Smart Card – Rs. 2,250/-
3. Registration of Add. Qualification + Smart Card – Rs. 1,250/-
Registration of Add. Qualification Other State + Smart Card – Rs. 1,350/-
4. Duplicate Reg. Certificate of Additional/Permanent Registration – Rs. 2,400/-
5. Good Standing Certificate – Rs. 5,000/-
6. NOC Certificate – Rs. 1,000/-
7. Dentists Renewal Fee (for Five year) 500/- Per Year – Rs. 2,500/-
8. Dental Hygienists / Mechanics Renewal Certificate (for Five year) 200/- Per Year – Rs. 1000/-
9. Duplicate Dentists Renewal Card Fee – Rs. 500/-
10. CDE Points Fee Per Day – Rs. 1000/-
11. New ID Card (Smart Card) – Rs. 250/-
12. Duplicate / Old ID Card (Smart Card) – Rs. 500/-
13. Dentists Prov. Registration Fee. – Rs. 1,000/-
14. Correction Fee – Rs. 1,000/-
15. Name Change Fee – Rs. 2,000/-
Note :
1 Rs. 100/- for Miscellaneous /Postal Charges & Rs. 25/- for DH/DM (only for Qualification from outside U.P.).
2 Rs. 10/- for Restoration Charges for Renewal after 31 march of the year.
Presence of Candidate is must for applying.

Please get the Colored Print out of Application Registration Form.
Print the Application Registration Form in A4 Paper Size only.

Please ensure that you submit the application ONLINE with in the stipulated date for that fee.
3. Your payment towards application fee will not make you eligible for registrations (Medical/ Nursing/ Paramedical etc). U.P.S.M.F. verifies your candidature as per the rules and guidelines and will decide to accept or reject your application.
It is the sole discretion of the U.P.S.M.F. committee to finalise the eligibility of the applicant for registrations (Medical/ Nursing/ Paramedical etc) as per the information provided by the applicant.
4. Any dispute arising out of U.P.S.M.F. shall only be settled and decided under the jurisdiction of Hon’ble Lucknow High Court, Lucknow.

    i search updental website but never got any link to about bds identity regarding

    only bds identity card fee detail avail over there but apply online process not avail there

    if you have online way then express the way
